Help AFHC recognize a caregiver whose compassion, reliability, professionalism, and commitment make a meaningful difference in an adult family home. Providers, coworkers, residents’ family members, and community partners are invited to submit a nomination. Self-nominations are also welcome.

Tell us what makes this caregiver exceptional and share a specific example of how they have supported residents, coworkers, or the adult family home community. Please protect resident privacy and do not include names, diagnoses, photographs, or other identifying information without written permission.

Eligibility: The nominee must currently provide caregiving services in a Washington-licensed adult family home.
