Adult Family Home Council Annual Membership Fee

Annual membership fees are paid once a year at $600 with an additional 4% credit card processing fee for a total of $624. If you would like to avoid paying a processing fee, we recommend paying your memberships through ACH, which will draw payment directly from your bank account. With ACH, you’ll never need to worry about your card expiring or forgetting to call if your card is lost or stolen.

To cancel your annual membership payment please send a written notice by email to info@adultfamilyhomecouncil.org or by mail to the Adult Family Home Council at 3309 Capitol Blvd SW, Tumwater, WA 98501. No refunds will be issued for past payments.

Description

Membership years for the Adult Family Home Council run from July 1-June 30 every year.

Membership in the Adult Family Home Council is $600 per year.  That comes out to $50 per month.  $50 per month is less than 1% of the average Medicaid payment.

Payment plans are available.  If you want to have $50 a month withdrawn from your account, or have the $50 deducted by the Provider One payment system, please complete these forms and return them to the office.  This method of payment also ensures no break in membership, and the loss of access to the website and the listing of your home on the locator.
