This course provides Adult Family Homes with practical knowledge regarding home health and hospice services, recognizing resident decline, facilitating goals-of-care conversations, and building collaborative partnerships with healthcare providers.  Participants will learn how early identification of changing resident needs can improve quality of life, reduce avoidable hospitalizations, and support residents’ end-of-life wishes while enhancing care coordination within the Adult Family Home setting.
