This educational program is a comprehensive review of hospice as a community partner for your adult family home (AFH). We will be discussing how a good hospice interdisciplinary team can keep your AFH in compliance with state regulations and get you better inspection outcomes. We will look at the services that come with hospice as well as the ways a good hospice partner can give comfort and solace to residents and families at the hardest time in every life.
Attendees will learn about the roles and responsibilities of the interdisciplinary team members within hospice. We will review what a hospice appropriate senior looks like when they are approaching end of life and emphasis will be placed on the clinical requirements for Medicare hospice eligibility. Finally, a discussion of when and how to best utilize hospice as a partner for your AFH as well as a refuge for your residents and the people who love them.
