Description
Empathy Haven Adult Family Home, located in Tacoma, WA is managed by Tackesha Thomas a Family Nurse Practitioner We are a licensed AFH (License #758193) with availability to accept private-pay residents immediately, and we are actively completing our DSHS contracting process to serve Medicaid clients as well.
Empathy Haven AFH offers compassionate, home-like care for:
• Dementia & Alzheimer’s patients
• Parkinson’s Disease
• Diabetes management
• Fall risk and mobility support
• Mental health and developmental disabilities
• Hospice and end-of-life care
Our home is licensed for up to 4 residents, and our focus is on personalized, medically informed care in a warm, family-centered environment.
Services
- 2 hour turns for mobility impaired
- 24/7 staff available
- ADMINISTRATION OF NUTRITION AND MEDICATIONS VIA A VARIETY OF FEEDING TUBES, INCLUDING, BUT NOT LIMITED TO, G-TUBES, J-TUBES, NG-TUBES
- ARNP owned and operated with 24/7 on call team available
- Activities to be added per client\'s assessments/needs

- Foley Care
