Lively Care Adult Family Home provides dependable, high-quality 24-hour care in a safe, comfortable home setting—ideal for discharges from hospitals, skilled nursing facilities (SNFs), and assisted living communities. We focus on smooth, timely transitions, ensuring residents receive attentive care and close monitoring during recovery.
Our experienced caregivers assist with activities of daily living, medication management, mobility support, and ongoing care coordination. We work closely with hospital discharge planners, SNF teams, assisted living staff, case managers, and families to support safe placements and continuity of care.
We accept both Medicaid and private pay and are committed to delivering compassionate, individualized care that promotes stability, dignity, and quality of life.
