About the Award
September is Adult Family Home Month—a time to honor the heart, soul, and hard work of those who care for our most vulnerable. In celebration, the AFH Council is proud to announce our 2025 Exceptional Caregiver Award—a moment to shine the spotlight on the extraordinary people who give so much of themselves every single day.
This is more than an award—it’s a tribute to the caregivers whose compassion, patience, and unwavering commitment transform lives. They are the ones holding hands through difficult days, celebrating victories big and small, and making every person they care for feel valued and loved. Let’s give them the recognition they so deeply deserve.
How to Nominate a Caregiver
To nominate a caregiver, simply fill out the form below. Share their name, the adult family home where they serve, their phone number, email, and—most importantly—a heartfelt story that shows the kindness, dedication, and love they pour into their work every day.
We’d also love for you to pass this form along to residents and their families—because sometimes the most touching nominations come from the people whose lives have been forever changed by a caregiver’s compassion.
Award Ceremony
All nominated caregivers and the winning caregiver will be honored at the Fall Conference. This will be an afternoon filled with heartfelt stories and a collective sense of gratitude for these extraordinary individuals.

Membership fees enable the Council to cover legal expenses and fund staff to advocate with the state and regulatory agencies. The participation of every adult family home is vital to ensuring fair regulations and rates that accurately reflect the costs of caring for our vulnerable adults.
