About the Award

The Governor has declared September as Adult Family Home Month, and to honor this occasion, the AFH Council is hosting an Award Ceremony to recognize exceptional caregivers in adult family homes. The nomination period for the 2024 Exceptional Caregiver award is now officially open! We urge all AFH providers, residents, their families, and colleagues to participate and nominate an outstanding caregiver.

This prestigious award celebrates the remarkable dedication, compassion, and selflessness of caregivers who tirelessly support and care for adults in need. These unsung heroes make a profound difference in the lives of vulnerable adults, providing exceptional care and unwavering support.

How to Nominate a Caregiver

To nominate a caregiver, simply click the “Nominate A Caregiver” button below and fill out the form. Please include the caregiver’s name, the name of the adult family home they work in, their phone number, email, and a heartwarming story that exemplifies their exceptional care and dedication. You can also send the form to your residents and their families to submit their nomination!

Award Ceremony

All nominated caregivers will be recognized at a hybrid award ceremony held a week before the AFH Council 2024 Fall Conference, with the winning caregiver honored at the Fall Conference. This will be a morning filled with heartfelt stories and a collective sense of gratitude for these extraordinary individuals.

The deadline for nominations is August 19, 2023. Don’t miss this opportunity to recognize and celebrate the exceptional caregivers who make a difference every day!
