We work seamlessly with Home Health, Hospice, or independently—always with the same goal: keeping patients safe and comfortable in their Adult Family Home (AFH), while reducing unnecessary readmissions to skilled nursing facilities or hospitals.

We understand how challenging it can be to transport patients to outside appointments. As proud supporters of the AFH community, we are committed to serving residents in long-term care right where they live.

We bill Medicare Part B and Medicare Advantage plans. Best of all, no orders or referrals are required—caregivers can call us directly from the home, we are an ADVANCED wound care, so we do have steps to follow.
