Medicaid-contracted adult family homes may receive a notice requiring them to confirm their provider information and credentials.

Providers who do not respond or complete the required process could face termination of their Apple Health enrollment or DSHS contract.

What is provider revalidation?

Revalidation is a required process used to confirm that an Apple Health provider’s enrollment information and credentials are accurate and meet state and federal requirements.

Providers ordinarily complete revalidation every five years. DSHS-contracted providers typically complete it when renewing their Medicaid contracts.

An off-cycle revalidation takes place outside that normal schedule.

What is changing?

The Centers for Medicare & Medicaid Services has directed HCA and DSHS to complete revalidations of Apple Health providers by June 5, 2028.

HCA and DSHS will contact providers who need to complete an off-cycle revalidation. Providers do not need to begin the process until they receive a notice.

Which DSHS providers may be affected?

A DSHS-contracted provider must complete off-cycle revalidation if the provider:

• Did not renew a contract between June 2025 and June 2026; and

• Is not scheduled to renew a contract between June 2026 and June 2028.

DSHS providers will need to complete the off-cycle process only once, even if they hold multiple contracts. The process will not create a new DSHS contract.

What providers should expect

DSHS will send affected providers an email containing:

• Required documents or information

• Completion instructions

• The applicable deadline

• Contact information for questions

HCA will separately contact affected HCA-enrolled providers using the email or mailing address listed in ProviderOne.

What you should do now

• Make sure your HCA and DSHS contact information is current.

• Monitor your email, spam folder, and postal mail for a notice.

• Read the notice carefully.

• Complete the requested revalidation by the stated deadline.

• Contact HCA or DSHS if you have questions about the request.

Payments will continue normally while a provider completes the off-cycle revalidation.

The bottom line

Do not ignore a revalidation notice. HCA may terminate a provider’s Apple Health enrollment for failing to complete the process, while DSHS may follow its standard contract-termination process.

Providers should act promptly when contacted and retain copies of the notice and submitted materials.
