
HCA’s Single-Vendor Supply Plan Raises Concerns
The big picture: The Washington State Health Care Authority (HCA) is moving forward with a plan to select one statewide mail-order vendor for incontinence and urological supplies provided to Apple Health fee-for-service clients.
Why it matters: These products are not one-size-fits-all. Adult family home residents may need specific sizes, absorbency levels, materials and delivery support to remain safe, comfortable and healthy.
A single-vendor system could mean:
Fewer product and supplier choices
Disruptions during the statewide transition
Longer waits when a resident’s needs change
Less access to local suppliers familiar with residents and providers
By the numbers: According to reporting from the Tacoma Daily Index, Washington served as many as 20,000 patients and paid suppliers more than $30 million for these products in 2024. HCA estimates consolidation could save approximately $4 million to $8 million. More than a dozen Washington suppliers told the publication that the change could seriously harm their businesses or force them to close.
What HCA says: The agency is pursuing the model to reduce costs, standardize service and encourage higher-quality products through value-based purchasing.
The concern: Cost savings should not come at the expense of resident choice, continuity of care or timely access to medically appropriate products.
Where it stands: This change has not yet taken effect. HCA’s competitive solicitation remains active, with proposals currently due Tuesday, September 15, 2026, and services scheduled to begin January 1, 2027.
AFHC’s position: AFHC is urging HCA to pause the single-vendor proposal and work directly with adult family homes, residents and existing suppliers on an approach that preserves multiple qualified vendors.
The bottom line: Washington can pursue savings while protecting resident dignity, safety and choice. Adult family homes must have a meaningful voice before this system is changed.
