RCS AFH Pilot for Facility Reported Incidents

Residential Care Services (RCS) launched a six‑month “proof of concept” pilot in Region 1 on July 1, 2026 to test an administrative review and offsite investigation process for select facility‑reported incidents (FRIs). The pilot applies to medium‑ and low‑level harm incidents that do not require an onsite investigation, allowing RCS to evaluate whether remote investigations are feasible, effective, and efficient. This pilot is focused specifically on Region 1. 

Facility‑reported incidents are events or allegations that adult family home providers must report to the state hotline as mandatory reporters. Under the pilot, providers will continue to submit initial FRI reports through the Residential Care Services Online Incident Reporting (OIR) portal, as they do today.

 

RCS Implementation Updates

Adult Family Home Council (AFHC) and RCS partnered on a joint training for adult family home providers and staff. This virtual session:

  • Provided an overview of the AFH FRI pilot and its objectives
  • Explained legal and regulatory requirements for mandatory incident reporting
  • Clarified expectations for submitting five‑day follow‑up updates
  • Offered step‑by‑step instruction on how to upload documents through the OIR portal

Additional resources:

    •  RCS is asking providers to fill out and submit this form in the reporting portal within 5 working days after their initial mandatory report.  Purpose is to provide an update on their mandatory-reported incident.  This form is being tested in the “proof of concept” pilot and are asking providers to partner in the pilot and help test it.  It is a fillable form.   RCS will post this form on the online incident reporting portal webpage on July 17, 2026 in time for launch of the pilot.
