NEW APPLICATION

  • Combines the current separate DOH and Prometric applications into one form.
  • Completed form is submitted to DOH; Prometric will continue to administer the exam.

  • Old applications received after April 29, 2016 are incomplete; applicants will receive an email requesting missing information.
  • Applicants who applied to DOH but not Prometric as of May 1, 2016, must complete an Examination Request Form and return to DOH with the exam fee (if applicable).
  • Contact Prometric directly to retake the exam.

NEW CERTIFICATION EXAM

  • Uses simplified language so the questions are easier to understand, but they are not easier.
