Continuum Care Hospice of Washington is a home hospice provider.  We care for patients in their home, wherever that may be, whether it is an Adult Family Home, a private residence, a community, or somewhere else entirely.  We provide these services in King, Pierce (as of October, 2022,) and Snohomish Counties.

Our care teams are made up of RN case managers, our Medical Director(s,) Home Health Aides, Spiritual Care Coordinators, certified Music Therapists, and Medical Social Workers. Because of this, our teams can care for a patient up to five times per week, and our services are covered by the Medicare Hospice Benefit.
